SCP provides a full spectrum of clinical services in our mission to empower each of our clients with 360º of care. We begin with a clinical assessment of each client as a first step in the care process. A Written Plan of Action for individual care is submitted for review and approval. SCP then executes the written plan with ongoing monitoring and continuous communication.

Dementia

Our goal is to sustain our client with emotional support while emphasizing independence and a sense of dignity with daily life activities.

  • We assess our client’s current living situation andany safety concerns, then implement and monitor in-home services.
  • The SCP team will then identify, as needed, long-term care facilities where programming and care for dementia clients are provided: i.e. reminiscence and social activities.
  • SCP will then set up a mutually agreed schedule for on going in-person monitoring.

Developmentally Delayed

SCP helps developmentally delayed individuals maximize their independence and lead more self-determined lives.

  • We begin a client relationship with an initial care management assessment for function, living and safety—then develop an individualized written care plan.
  • Upon approval, the care plan is implemented.We ensure regular school attendance or healthy work environment (as needed).
  • SCP will then set up a mutually agreed schedule for on going in-person monitoring.

Traumatic Brain Injury

SCP supports our clients with TBI maximize independence and maintain dignity throughout activities of daily life.

  • We begin a client relationship with an initial care management assessment for function, living and safety—then develop an individualized written
    care plan. Upon approval, the care plan is implemented.
  • SCP will then set up a mutually agreed schedule for on going in-person monitoring.

Physically Challenged

  • SCP provides referrals to appropriate services and resources.
  • We provide assistance with benefit coordination, home care, waiver services and residential placement.
  • SCP will then set up a mutually agreed schedule for on going in-person monitoring.

Autism Spectrum

We begin a client relationship with an initial care management assessment for function, living and safety—then develop an individualized written care plan.

  • Upon approval, the care plan is implemented. SCP provides behavioral intervention, supportive services and caregiver support.
  • We also locate and secure appropriate resources, schooling, and benefits.
  • SCP will then set up a mutually agreed schedule for on going in-person monitoring.

Mental Health Issues

SCP helps determine eligibility for services, beginning with a bio-psychosocial assessment and social history research.

  • We begin a client relationship with an initial care management assessment for function, living and safety—then develop an individualized written care plan.
  • Upon approval, the care plan is implemented.
  • We then assess clients for substance abuse, support systems, physical and emotional function, financial stability and general safety.
  • SCP provides crisis management and family intervention.
  • We provide ongoing care management services such as referrals to community resources and collaboration with other care professionals.
  • SCP will then set up a mutually agreed schedule for on going in-person monitoring.

Special Needs

The highly experienced SCP staff work with trust officers and with Special Needs Trusts to comply with DPW and Social Security regulations.

  • We begin a client relationship with an initial care management assessment for function, living and safety—then develop an individualized written care plan.
  • Upon approval, the care plan is implemented.
  • Other services include documentation of needs, court evaluations and court recommendations for care management.
  • SCP will then set up a mutually agreed schedule for on going in-person monitoring.

Intellectual Disabilities

SCP assists families in registration with County Office of Intellectual Disabilities to obtain care management services for our clients.

  • We begin a client relationship with an initial care management assessment for function, living and safety—then develop an individualized written care plan.
  • Upon approval, the care plan is implemented.
  • SCP assists in securing home care, residential placement, waiver services and benefit coordination.
  • SCP will then set up a mutually agreed schedule for on going in-person monitoring.

Visually Impaired/Deaf/Hard of Hearing

SCP provides interpretation services, audiological services and devices, access to Braille products and adaptive equipment.

  • We begin a client relationship with an initial care management assessment for function, living and safety—then develop an individualized written care plan.
  • Upon approval, the care plan is implemented.
  • We provide care management services at schools in Philadelphia and Delaware County that specialize in serving Visually Impaired, Deaf and Hard of Hearing students and their families.
  • We act as client advocate for all applicable state or county benefit services.
  • SCP will then set up a mutually agreed schedule for on going in-person monitoring.
