Family Members

SCP’s team of care management professionals have extensive experience working with family members for the care of loved ones. SCP provides: 

• Initial assessment and a detailed care plan with recommendations for services.

• Transition of a family member from home to assisted living, nursing home, etc.

• Home support coordination.

• Supportive care to maintain a family member’s independence in their own home

• Regularly scheduled well being and proper care check-ins.

We also work with professionals and care facilities.
